To update your address, you must submit the change in writing.

How to submit your address change

1. Fax: Send your updated address information to 1-888-265-2144.
2. Email: Email your address change request to [e-mail].

What to include in your request

• Your full name.
• Your reference number.
• Your old address and your new address.
• A clear request to update your address in the system.
